I like #1 a lot. And it doesn't even have to be a monster nor used a lot if done right the first time.
Remember reading Treasure Island in middle school? Remember "The Black Spot" that marked someone for death? If a character receives a "death mark" then the player should naturally be a little paranoid of when, where, and how it will play out. A few suspicious, and possibly just coincidental, occurrences can increase the paranoia and tension.
